How the referral process works

Referrals

The referral pathway ensures patients receive safe, appropriate exercise support alongside their medical care, helping maintain independence, confidence, and quality of life.

Referrals often begin during a routine appointment, where a healthcare professional discusses the benefits of physical activity for recovery, strength, or managing treatment side effects.

With the patient’s consent, a referral is made by securely sharing relevant medical information and any necessary exercise considerations to support safe, personalised guidance.

Consultation

Once the referral is received, the personal training service contacts the patient directly to arrange an initial consultation.
This first appointment includes:

  • A detailed health and lifestyle discussion
  • Review of medical considerations and current treatment
  • Screening for safety and any exercise restrictions
  • Goal setting based on the patient’s needs and priorities

Following this assessment, a personalised exercise programme is developed. This may include prehabilitation, support during treatment, or rehabilitation afterwards, with all sessions adapted to the individual’s condition, energy levels, and functional ability.

Communication

With your consent, OAK Personal Training can communicate with your healthcare team to ensure your exercise programme is safe, appropriate, and aligned with your medical care. All information is shared securely and confidentially, supporting a joined-up approach that helps you exercise with confidence and peace of mind.
